A.  For ALL APPLICANTS

_____    ASI Form No. 6 (application form)   two (2) copies
_____    Official Transcript of records (Original copy with Special Order)
_____    Honorable Dismissal / Transfer Credential from the last school attended

Foreigners should have their transcript of records authenticated by the Philippine Embassy or Consulate in their country of origin or legal residence. “SEEN and NOTED” stamp is not acceptable.

_____    Diploma or certificate of completion of any bachelor’s degree
_____    Two (2) letters of recommendation from the agency or organization the applicant is connected with
_____    Short autobiography (see below for the guide)
_____    2 passport-size pictures
_____    passed the entrance test
_____    passed the interview

B.    Additional requirements for FOREIGN APPLICANTS

_____    Personal History Statement (Revised Form 1998) Six (6) Copies

_____    Xerox copy of Passport pages where name, photo, birth date, and birthplace appear

_____    A notarized affidavit of support AND proof of adequate financial report to cover expenses for student’s accommodation and subsistence, school dues and other incidental expenses.  For scholars, certificate of scholarship from the institution concerned.

_____    Certification of good health

_____   English language proficiency (oral and written)
