Admission Requirements

For All

Applicants


Documents to be submitted:

  • formal letter of application
  • ASI Form No. 6 (application form) two (2) copies
  • Official Transcript of records (Original copy with Special Order)

Foreigners should have their transcript of records authenticated by the Philippine Embassy or Consulate in their country of origin or legal residence. “SEEN and NOTED” stamp is not acceptable.

  • diploma or certificate of completion of any bachelor’s degree
  • two (2) letters of recommendation from the agency or organization the applicant is connected with
  • short autobiography (see below for the guide)
  • 3 passport-size pictures (taken within 6 months) during application period
  • passed the entrance test
  • passed the interview

Give an account of your life by discussing the items below. It would be interesting if you could discuss them in detail. There is no limit in the number of words.

  • Philosophy of life – beliefs, ideas, convictions
  • Experience – turning points, crises, highlights, and important events
  • Family background
  • Significant people in life
  • Involvement with groups and communities

Additional requirements:

  • Personal History Statement (Revised Form 1998) Six (6) Copies
  • Xerox copy of Passport pages where name, photo, birthdate, and birthplace appear
  • A notarized affidavit of support AND proof of adequate financial report to cover expenses for student’s accommodation and subsistence, school dues and other incidental expenses. For scholars, certificate of scholarship from the institution concerned.
  • English language proficiency (oral and written)

To be submitted to the Philippine Embassy in your country when you pick up your student visa:

  • Certification of good health/Medical Certificate
  • Police Clearance
